Physiotherapy after knee replacement: what recovery really looks like

The surgery is the easy part. What you do over the following months is what determines your result.

In short

Recovery after a knee replacement is driven by structured physiotherapy: gentle movement and walking begin within days, most people walk comfortably and climb stairs within 6 weeks, and full strengthening continues for 3–4 months. Following your surgeon's protocol and doing your exercises consistently are the biggest factors in a good outcome.

Why rehab matters more than the surgery

A knee replacement gives you a new joint, but it can't give you back the strength, movement and confidence you lost in the months of pain beforehand. That's the job of rehabilitation — and how well you do it largely determines how good your new knee feels.

A realistic timeline

Everyone heals at their own pace, and your surgeon's protocol comes first, but a typical journey looks like this:

  1. Week 1–2Gentle movement, swelling control and walking with support. Early bending and straightening exercises begin almost immediately.
  2. Week 3–6Walking distance increases, you wean off walking aids, and you work toward bending the knee past 90 degrees and straightening it fully.
  3. Week 6–12Strengthening steps up. You return to most daily activities, stairs become easier, and balance work begins.
  4. Month 3–6Continued strength and function work, returning to activities like longer walks, cycling and swimming.

The milestones that matter

  • Straightening the knee fully (full extension) — important for a normal walking pattern
  • Bending past 90 degrees, then toward 110–120 for stairs and sitting
  • Walking without a limp and without aids
  • Rebuilding quadriceps and glute strength to protect the joint

How to avoid setbacks

The two most common mistakes are doing too much too soon, and doing too little out of fear. A physiotherapist keeps you in the productive middle — progressing your exercises at the right pace, managing swelling, and catching problems early. For the first weeks, when travelling is hard, home physiotherapy can be the easiest way to stay consistent.

Frequently asked questions

How soon after knee replacement does physiotherapy start?

Usually within a day or two — early movement is a key part of modern recovery. Your physiotherapist follows your surgeon's protocol for timing and progression.

Is physiotherapy after knee replacement painful?

Some discomfort is normal, especially early on, but rehab should be challenging rather than sharply painful. Your therapist works within a safe, tolerable range and helps you manage swelling and soreness.

Can I do knee-replacement rehab at home?

Yes. Home physiotherapy is ideal in the early weeks, and we offer it across Domlur and South-East Bengaluru so you can recover safely without travelling.
